Collecting Information Through Web Forms
Online forms are quickly replacing paper and faxed forms as a popular way to collect information from people visiting your website. Collecting information online can in many cases eliminate extra work, save paper and speed up processing. Creative Services is happy to assist your program with steamlining the data collection process. Please understand that creating online forms is a complicated process, and as such requires at least one month advance notice in order to meet deadlines.
While web forms can make gathering data faster and more convenient, they can also make it easier for your information to fall into the wrong hands.
In order to protect privacy and prevent data theft, Creative Services cannot collect the followings types of information online:
- Social Security Number
- Date of Birth
- Student ID Number
- Credit Card / Procurement Card Information
- Maiden Name
- Medical Information
- Banking Information or Account Numbers
- Criminal History Information
- Scholastic Records, such as GPA and test scores
- Marital Status
- Any collection of data that, in aggregate, could adversely effect an individual's privacy if compromised.
Creative Service can only collect data that is publicly available or considered to be "directory information".
